ACCEPTED PAYMENTS

Accepted Payments

We accept cash, Lava gift cards, and all major credit cards. We do not accept third-party gift cards.

Lava also accepts HSA, FSA, and HRA card payments.

If you have an HSA (Health Savings Account), FSA (Flexible Spending Account) or HRA (Health Reimbursement Arrangement) your debit card will work here, and your session (not including tips, which should be paid separately) will be considered an eligible and reimbursable medical expense. A written prescription or letter of medical necessity from your doctor is required to book a medical massage.

Examples of illness that could qualify for medical massage treatment include anxiety and depression, arthritis, back pain, carpal tunnel syndrome, chronic fatigue syndrome, diabetes, fibromyalgia, hypertension, injury/surgery, pain management, and stress.

If you have been suffering from any of these conditions, let your doctor know that you are participating in an HSA, FSA, or HRA plan, and that you would like to use some of your funds for medical massage treatment.

On the prescription or letter of medical necessity, (which we will keep a copy of for our records, but you should file in case you are asked to justify the expense) your doctor should indicate:

1. The diagnosis of the condition requiring treatment.

2. The primary purpose of the therapy (ex: to relieve stress, help alleviate back pain, etc.)

3. The suggested frequency and duration of the treatments (ex: weekly sessions for at least 6 months.)

NOTE: Funds contributed to an HSA are generally exempt from federal income tax. You are permitted to take a distribution from your HSA at any time; however those amounts used exclusively to pay for qualified medical expenses are tax free. If you have questions about coverage, please call the phone number on the back of your card and talk to your program administrator.

Terms and Conditions for Lava Massage and Skincare Gift Cards:
-All gift card sales are final. Gift cards cannot be redeemed for cash.
-Promotional gift cards sold at a discounted price cannot be combined with any other specials, coupons, discounts, or packages, and cannot be used to purchase retail products.

Health/Medical Insurance: We do not accept health insurance as payment and we are unable to bill your insurance provider. If medical massage is a covered treatment under your health insurance plan, please contact your insurance provider for more information and instructions for reimbursement. What we can do is give you a receipt or printout out of your massages to submit to your insurance company for them to reimburse you back.
