Benefits of becoming a member

• Unused services roll over to the following month as long as you remain a current and active member.
• No waiting to check out after your massage. Your credit/debit card will automatically be charged each month.
• Guaranteed the best deal--you will not find a better price on a quality, customized therapeutic massage anywhere else. We strive for excellence at Lava Massage and our quality of service is impeccable. We believe we have the best and most talented Licensed Massage Therapists in the State of Ohio.

Features and fine print

• Membership is a 12-month agreement and will renews automatically on a month-to-month basis after the first 12 months.
• Payment made via auto-debit once a month according to which membership you may choose. Sales tax will be added to the price of each service (except for medical massage with a doctor's prescription).
• Membership can’t be combined with any other offers or discounts.
• Appointments must be cancelled with 24 hours notice to avoid cancellation fees (we need time to fill in to replace your appointment with someone else & if we cannot, then the fee partly compensates the therapist for their time lost).
• To cancel (or modify) your membership you must complete & sign a Lava Massage & Skincare membership Cancellation Request Form which is available at front desk, or send us via email, certified letter or a written statement.
• If you cancel your membership during the first 12 months you will be charged a cancellation fee of $175. There is no cancellation fee if you cancel after the first 12 months.
  
• Failure to pay for 2 consecutive months of the membership will automatically terminate your membership and all remaining massages will be forfeited.
• You may freeze your account for up to 3 months within any 15 month period.
• No credits or refunds will be issued for no shows, last minute cancellations or late appointments.
• You may upgrade your membership to a higher level at any time.
